How This Form Is Used

• The Maintenance Department begins the process by describing the particular problem, and that information is printed on the form.
  
  
• Next, depending on the particular problem, one or more members of the maintenance staff completes the necessary work.

• Once the work is completed, the maintenance supervisor, the originator of the request and the Maintenance Accounting Department receive hard copies of the form.
