- The Maintenance Department begins the process by describing the particular problem, and that information is printed on the form.
- Next, depending on the particular problem, one or more members of the maintenance staff completes the necessary work.
- The maintenance work is recorded, and the bar code is scanned.
- Once the work is completed, the maintenance supervisor, the originator of the request and the Maintenance Accounting Department receive hard copies of the form.
- The bar code makes it possible to have a tracking record of the maintenance work. The bar code also allows for highly accurate recording of information, which is critical to ISO 9000 compliance.
